Through the Collaborative Care Model, an evidence-based model for treating behavioral health issues in primary care, April Health enables access to behavioral health services through doctors that patients already know and trust.
April Health partners with primary care, women's health, and pediatric practices to provide a hybrid behavioral health solution that combines human care management with AI-driven support to improve patient outcomes, reduce provider burden, and generate new revenue for our partners. Patients conduct regular appointments with our behavioral health care managers, receive guidance from psychiatrists, and our AI Therapeutic Coach helps patients between appointments, maximizing the value that patients receive in our program.
Position Overview
We are hiring a Patient Enrollment Lead to own and scale April Health's enrollment function. This is a true player-coach role, and it's important to be clear about the sequencing: you will not be managing a team on day one. In the near term, you will be the enrollment function — hands-on in the queue, making patient calls, scheduling intakes, and learning our workflows from the inside. From day one, you will also be accountable for enrollment performance: conversion, time-to-intake, no-show rates, and the patient experience. Then, as patient volume grows, you will earn the opportunity to hire, train, and lead a team of Enrollment Coordinators under you, building the systems, guidebooks, and analytics that allow enrollment to scale.
This role is ideal for someone entrepreneurial — a builder who is energized by doing the work first, improving it second, and then teaching others to do it at scale. You'll work directly with company leadership and have outsized influence on how a core function of a fast-growing behavioral health company is designed.
What You'll Do
Hands-On Enrollment (your primary focus initially)
- Own the patient referral and outreach queue: monitor multiple EHR systems (e.g., AthenaOne, TouchWorks, Epic) for new referrals, conduct high-volume outbound patient calls, and complete associated data entry
- Serve as the first point of contact for referred patients, educating them about April Health’s Collaborative Care and guiding them through enrollment with warmth and professionalism
- Communicate with patients via phone, SMS, and email; conduct rescheduling and re-engagement outreach for unreached or disengaged patients
- Handle complex or escalated patient conversations with confident de-escalation and compassionate communication, following safety protocols for patients in crisis
- Collaborate with healthcare providers and internal care teams to ensure seamless coordination and prompt updates on patient status
Enrollment Performance & Process Improvement (ongoing)
- Own enrollment KPIs: conversion funnel, time-to-intake, conversion to completed appointment, no-show trends, channel effectiveness, and enrollment NPS — with monthly reporting to leadership on performance and areas of opportunity
- Continually refine enrollment workflows, scripts, and templates to improve efficiency, reliability, and patient experience
- Propose and test multi-channel engagement strategies (SMS, email, calls, AI-assisted outreach) to improve conversion
- Partner with product/tech and external vendors to evaluate and implement tools that support enrollment automation and scheduling
- Document and maintain SOPs, keeping the Enrollment Guidebook and training materials onboarding-ready at all times
Team Building & Leadership (future phase — no direct reports initially)
This role starts as an individual contributor. As enrollment volume grows and the business case supports it, you will:
- Hire, train, and manage Enrollment Coordinators, ensuring operational consistency and high-quality patient experience
- Conduct regular 1:1s, coaching, and shadowing to develop team members
- Set team targets and OKRs, and foster a culture of continuous improvement where the team surfaces and tests new ideas
- Remain hands-on in the queue as needed, including coverage during team absences
Qualifications & Experience
Required
- 3+ years of experience in healthcare operations, patient access, enrollment, or a related patient-facing role
- Demonstrated track record of process improvement: you've built or meaningfully improved a workflow, guidebook, or system, not just executed one
- Comfort with high-volume phone-based patient outreach, including with patients experiencing behavioral health needs
- Strong analytical skills: comfortable owning a metrics dashboard, identifying trends, and turning data into action
- Proficiency with EHR/CRM systems and Google Suite
- Entrepreneurial mindset with both comfort and experience operating in ambiguity within a fast-paced, remote startup environment
- Dedicated quiet workspace with high-speed internet (100+ Mbps download / 20+ Mbps upload)
Preferred
- Experience in behavioral health settings or familiarity with the Collaborative Care Model
- Prior experience hiring, training, or managing a small team
- Bilingual — fluent in Spanish and English (spoken and written)
- Experience evaluating or implementing patient engagement technology (SMS platforms, scheduling tools, AI-assisted outreach)
Expected Work Schedule
- Full-time, 40 hours per week
- Fully remote with availability during standard U.S. business hours
